Evergreen Counseling & Consulting is a private practice fee for service. The only health insurance that is accepted at our practice is Blue Cross Blue Shield Health Insurance. You are responsible for the copay, co-insurance and/or any deductible that has not been met.

If you have out-of-network benefits, you have the option to submit your claim to your health insurance company claiming on your out-of-network health benefits.
